About maximum lidocaine dose

Lidocaine is a local anesthetic used for infiltration, nerve blocks, and many procedural applications. Estimating a maximum dose helps clinicians limit systemic exposure, but the calculation is only one part of safe administration. Dose should reflect the procedure, injection site, vascularity, patient condition, other local anesthetics, and the ability to monitor and treat toxicity. This calculator applies commonly referenced adult limits of 4.5 milligrams per kilogram for plain lidocaine, capped at 300 milligrams, and 7 milligrams per kilogram for lidocaine with epinephrine, capped at 500 milligrams. It then converts milligrams to volume using the selected concentration. A 1% solution contains 10 milligrams per milliliter, 0.5% contains 5 milligrams per milliliter, and 2% contains 20 milligrams per milliliter. The lower of the weight-based amount and absolute cap is displayed. Epinephrine can reduce systemic uptake and prolong local action in suitable tissue, which is why a higher conventional limit is often cited. That does not make the higher amount appropriate for every patient or injection site. Product labeling, institutional protocols, specialty guidance, and patient-specific contraindications take priority. Pediatric, frail, pregnant, elderly, hepatic, and cardiac patients may require lower limits or different methods. Continuous infusions and intravenous antiarrhythmic use are outside this calculator's scope. Local anesthetic systemic toxicity can affect the central nervous and cardiovascular systems. Early findings may include circumoral numbness, metallic taste, tinnitus, agitation, or confusion, followed by seizures, altered consciousness, hypotension, conduction problems, or cardiac arrest. Prevention includes incremental injection, aspiration when appropriate, dose tracking, careful monitoring, and immediate access to resuscitation resources and lipid emulsion according to local protocols. Add every lidocaine source before comparing the total with a limit, including topical products, mixtures, repeat injections, and doses administered by other clinicians. A mathematically permissible maximum is not a target dose; use the smallest effective amount. Why is there an absolute dose cap?

Does epinephrine always permit 7 mg per kilogram?
