About the NEDOCS calculator

NEDOCS stands for National Emergency Department Overcrowding Scale. It combines seven operational measurements into one score intended to describe crowding at a particular moment: the number of patients in the emergency department, licensed or staffed emergency beds, admitted patients still in the department, hospital beds, ventilated patients, the longest boarding time, and the longest waiting-room time. The resulting number gives clinicians and administrators a common language for discussing pressure on space, staffing, and patient flow. The calculator applies the published linear model: negative 20, plus 85.8 times the ratio of emergency patients to emergency beds, plus 600 times the ratio of admitted emergency patients to hospital beds, plus 13.4 for each ventilated patient, plus 0.93 times the longest admit duration in hours, plus 5.64 times the longest waiting-room duration in hours. The displayed value is rounded to the nearest whole number and cannot fall below zero. Conventional bands describe conditions from not overcrowded through disaster-level overcrowding. Consistency matters more than precision beyond the model. Collect every input at the same time and use the bed definitions adopted by the organization. A staffed treatment space may differ from a licensed bed, and hospitals may define boarding or waiting intervals differently. Changing definitions between observations can create apparent improvement or deterioration that reflects data collection rather than actual operations. NEDOCS is most useful as one point in a trend and alongside local indicators such as left-without-being-seen rates, ambulance offload delays, staffing levels, and inpatient discharge capacity. A high score should trigger the facility's established escalation and surge procedures rather than an isolated response based only on this page. The scale cannot describe every safety threat, and a low or moderate value does not guarantee that a particular patient or care area is safe. Clinical acuity, infection-control needs, staffing mix, equipment availability, and local layout can all change risk.
