About the frailty index

A frailty index describes the proportion of a defined set of health deficits that are present in an individual. Deficits may include symptoms, diseases, functional limitations, cognitive findings, mood concerns, sensory problems, and laboratory abnormalities. The deficit accumulation approach recognizes that vulnerability generally rises as multiple small problems collect across physiological systems. It differs from phenotype tools that focus on a smaller group of features such as weakness, slowness, exhaustion, weight loss, and low activity. The calculation is straightforward: divide the number of deficits present by the number actually assessed. If six deficits are present among forty assessed, the index is 0.150, or 15 percent. A complete instrument may score some variables as zero, one-half, or one rather than using only whole counts. This simplified calculator accepts whole-number totals, so partial values should first be combined according to the chosen instrument's published rules. For an interpretable result, the deficit list must be sufficiently broad, cover several organ systems, become more common with age without being nearly universal, and be applied consistently. Missing data reduce the denominator. Researchers commonly use at least thirty to forty candidate deficits, though shorter validated instruments exist. Comparing scores from different lists requires caution because selection, coding, and missing-data rules can change the value. The displayed categories use broad educational bands: below 0.10 as robust, 0.10 through less than 0.25 as prefrail, and 0.25 or above as frail. Cutoffs vary by setting and purpose, and risk changes continuously rather than abruptly at a boundary. Age and sex provide context but do not alter the arithmetic ratio. A clinician should interpret the index together with function, cognition, social support, acute illness, medications, goals, and baseline trajectory. Frailty is clinically useful because it can identify vulnerability to falls, hospitalization, delirium, disability, prolonged recovery, and treatment complications. It is not necessarily permanent. Exercise, adequate nutrition, medication review, rehabilitation, management of reversible illness, and social support may improve resilience. The result should prompt a comprehensive geriatric assessment when appropriate, not label a person or deny beneficial treatment. Urgent new weakness, confusion, falls, or inability to manage daily needs requires timely medical evaluation.
