About the MIPI score

The Mantle Cell Lymphoma International Prognostic Index, or MIPI, is a disease-specific prognostic model developed for people diagnosed with mantle cell lymphoma. It combines four pretreatment factors that independently correlated with overall survival in the original clinical-trial population: age, Eastern Cooperative Oncology Group performance status, serum lactate dehydrogenase relative to the laboratory upper limit of normal, and white blood cell count. The result separates patients into groups with different outcomes at a population level. It does not predict exactly what will happen to one person and does not determine treatment by itself. This calculator uses the continuous MIPI equation. The age term is 0.03535 multiplied by age in years. The ECOG term is 0.6978 multiplied by performance status. The LDH term is 1.367 multiplied by the natural logarithm of measured LDH divided by the laboratory upper limit. The WBC term is 0.9393 multiplied by the natural logarithm of the count expressed as 10⁹/L. Adding those terms gives the classic MIPI score. Values below 5.7 are categorized as low risk, values from 5.7 to below 6.2 as intermediate risk, and values of 6.2 or higher as high risk. LDH must be normalized because reference ranges differ between laboratories. Entering only an absolute LDH value can misclassify the result. WBC units also matter: a result reported as 8,000 cells per microliter corresponds to 8 in 10⁹/L. ECOG should reflect the clinician-assessed baseline before treatment. A score of zero describes full activity, while higher values represent increasing functional limitation. Inputs collected during acute illness or after treatment may not reproduce the intended baseline prognostic estimate. The biologic MIPI shown here adds 0.02142 multiplied by the Ki-67 proliferation index to the continuous MIPI. Ki-67 is assessed on lymphoma tissue and provides information about tumor proliferation, but pathology methods and sampling can influence the percentage. The displayed biologic score is supplementary and the classic risk label remains clearly identified. MIPI is intended for use as one part of a comprehensive hematology assessment. Disease morphology, TP53 status, other molecular findings, comorbidities, fitness, symptoms, treatment response, patient goals, and available therapies also matter. Survival figures from historical cohorts may not reflect outcomes with current treatment. Clinicians should apply contemporary guidelines and multidisciplinary judgment rather than using a calculator result as a stand-alone treatment recommendation.
