About ECG box timing

Electrocardiogram paper turns horizontal distance into time. The grid is made of small 1 millimeter boxes, with every group of five small boxes forming one large 5 millimeter box. When paper runs at the common speed of 25 millimeters per second, one small box represents 0.04 seconds and one large box represents 0.20 seconds. At 50 millimeters per second, the same boxes represent 0.02 seconds and 0.10 seconds. This calculator uses the selected speed rather than assuming every tracing has the same calibration. To obtain the interval, the calculator first converts large boxes to millimeters by multiplying them by five, adds any small boxes, and divides the total horizontal distance by paper speed. If the measured span is an R-R interval, dividing 60 by the interval in seconds gives the equivalent heart rate in beats per minute. The heart-rate result is meaningful only when the measured boxes represent the time between comparable consecutive beats. Other ECG intervals, such as PR, QRS, or QT, should be interpreted as durations rather than heart rates. Check the printed calibration before measuring. Most recordings state 25 mm/s or 50 mm/s near the beginning of the strip, and a calibration pulse usually confirms voltage scale separately. Count from the same point on one waveform to the same point on the next. For R-R timing, using the peaks of two consecutive R waves is customary. If a boundary falls between grid lines, enter a decimal box count rather than rounding too early. With an irregular rhythm, one interval can differ substantially from the overall rate, so averaging multiple intervals or using a longer-strip method is preferable. This conversion supports ECG study and measurement but does not diagnose an arrhythmia or replace clinical interpretation. Paper distortion, screen scaling, poor signal quality, sweep-speed changes, and incorrect landmark selection can all alter the result. Clinicians interpret rate together with rhythm regularity, wave morphology, symptoms, medications, and the complete tracing. Seek urgent medical care for chest pain, fainting, severe breathlessness, or a sustained very fast or slow pulse. Use calculated measurements within your training and local clinical procedures.
