About ECG heart-rate calculation
An electrocardiogram records cardiac electrical activity against a calibrated time grid. Heart rate can be estimated from the R-R interval, the horizontal distance between the peaks of two consecutive QRS complexes. At the standard paper speed of 25 millimeters per second, 25 small boxes equal one second, so the rate is 1,500 divided by the number of small boxes. At 50 millimeters per second, the corresponding numerator is 3,000. This calculator expresses the same relationship as paper speed multiplied by 60, divided by the average small-box count. A single R-R interval gives an instantaneous rate. Entering several intervals lets the calculator average their box counts before calculating beats per minute, which is generally more representative when normal beat-to-beat variation is present. Count each interval from the same landmark, usually the apex of one R wave to the apex of the next. Separate measurements with commas, spaces, or semicolons. Confirm whether the tracing was recorded at 25 or 50 mm/s because selecting the wrong speed changes the answer by a factor of two. For a regular rhythm, the familiar large-box shortcut gives an approximate rate of 300 divided by large boxes at 25 mm/s. Counting small boxes provides finer resolution, especially when an R wave does not align with a heavy grid line. For an irregular rhythm, averaging only a few intervals may still misrepresent the overall ventricular rate. A longer rhythm strip can instead be timed over six or ten seconds and the QRS complexes scaled to one minute. Premature beats, pauses, artifacts, and paced complexes also require thoughtful interval selection. The numerical rate is only one element of ECG interpretation. It does not identify rhythm origin, conduction abnormalities, ischemia, or the clinical cause of tachycardia or bradycardia. Verify the recording calibration and assess the full tracing in clinical context.
