About ultrasound bladder volume

Bladder volume can be estimated noninvasively from ultrasound measurements. Because a filled urinary bladder is approximately ellipsoidal rather than rectangular, simply multiplying its three dimensions would overstate the volume. The commonly used approximation multiplies maximum length, width, and height by an ellipsoid correction coefficient of 0.52. When all dimensions are measured in centimeters, the numerical result is in cubic centimeters, which is equivalent to milliliters. Length and height are usually obtained in a sagittal view, while width is measured in a transverse view at the widest visible point. Measurement planes should pass through the bladder's greatest dimensions and calipers should be placed consistently according to the device and local protocol. Irregular shape, pelvic anatomy, bowel gas, probe pressure, and operator technique can all affect the estimate. Dedicated bladder scanners may use proprietary multi-plane algorithms and therefore may not match a manually calculated three-dimension estimate exactly. The clinical meaning depends strongly on when the scan was performed. A pre-void measurement estimates the amount present before urination and can provide context for bladder capacity or the urgency to void. A post-void residual measurement is taken soon after urination to estimate how much remains. The same volume can therefore have very different implications in a comfortably filling bladder and in a bladder that was just emptied. Record timing, symptoms, voided amount, and relevant medications with the measurement. This calculator labels measured volumes below 50 mL as minimal, 50 through 400 mL as moderate, and above 400 mL as large. Those broad labels describe the amount measured; they are not diagnostic thresholds. Post-void residual guidance varies with age, symptoms, setting, repeated measurements, and the clinical question. A single unexpected reading is often repeated with attention to timing and technique before conclusions are drawn. Ultrasound volume is one part of urologic assessment. Clinicians may also consider urinary symptoms, fluid intake, renal function, infection testing, neurologic history, prostate or pelvic conditions, and measured urine output.
