About the Apgar score

The Apgar score is a rapid, standardized description of a newborn's condition after birth. It is named for physician Virginia Apgar and also forms a useful mnemonic: Appearance, Pulse, Grimace, Activity, and Respiration. Each component receives zero, one, or two points, producing a total from zero to ten. The assessment helps the delivery team communicate how the baby is adapting and whether continuing support and reassessment are needed. A score is routinely documented at one minute and five minutes after birth. If the five-minute score remains below seven, many guidelines recommend repeating it at five-minute intervals up to twenty minutes while care continues. Each total belongs to its specific time point; values from different times should not be mixed. The score records the newborn's response, but resuscitation must begin whenever clinically indicated and must never be delayed while someone completes the scoring exercise. Appearance describes color, pulse records heart rate, grimace reflects response to stimulation, activity evaluates muscle tone, and respiration describes breathing effort. A total from seven through ten is generally considered reassuring. Four through six is moderately abnormal, while zero through three is low. These broad groupings support communication rather than serving as a stand-alone diagnosis. Gestational age, maternal medication, congenital conditions, infection, trauma, and resuscitation can influence individual components. The Apgar score was not designed to predict a particular child's intelligence, disability, or long-term neurologic outcome. A low score can have many causes and may improve quickly with appropriate support. Conversely, a reassuring score does not exclude every neonatal problem. Population studies may find associations between persistently low scores and later outcomes, but those associations cannot provide a certain prognosis for one infant. This calculator simply adds the selected observations. It is intended for education, documentation checks, and trained clinical use. Actual newborn evaluation requires direct assessment by qualified professionals following neonatal resuscitation protocols. When recording a score, include the assessment minute, any resuscitative interventions, and the component values rather than documenting only the total.

When is the Apgar score measured?

It is routinely measured at one and five minutes after birth. A score below seven at five minutes is commonly reassessed at five-minute intervals while care continues.

What does an Apgar score of seven or more mean?

A total from seven through ten is generally reassuring at that time point. It does not exclude every medical issue, so routine newborn assessment still continues.

Should resuscitation wait for the score?

No. Required warming, ventilation, compressions, or other neonatal resuscitation steps begin immediately based on clinical findings. Scoring documents condition and response but does not control the start of care.

Can Apgar predict long-term development?

It was not designed to predict an individual child's intelligence or neurologic outcome. Persistently low scores may inform population-level risk but are not a certain prognosis.

Does prematurity affect the score?

Yes. Premature infants may have lower tone, color, and respiratory scores because of immaturity rather than an acute birth event. Gestational context is essential when interpreting the result.
