About the LRINEC score

The Laboratory Risk Indicator for Necrotizing Fasciitis, commonly called LRINEC, combines six routine blood test results into a score intended to help distinguish necrotizing fasciitis from other severe soft tissue infections. It assigns points for C-reactive protein, white blood cell count, hemoglobin, sodium, creatinine, and glucose. Higher values reflect a laboratory pattern that was associated with necrotizing infection in the original derivation cohort. The scoring rules award four points when CRP is at least 150 mg/L. White blood cell count contributes zero, one, or two points; hemoglobin contributes zero, one, or two; low sodium contributes two; elevated creatinine contributes two; and elevated glucose contributes one. A total below six is commonly described as low risk, six or seven as intermediate risk, and eight or more as high risk. Those categories summarize the numeric result but are not probabilities for an individual patient. LRINEC has important limitations. External studies have reported variable sensitivity, especially early in disease, and a low score cannot safely rule out necrotizing fasciitis. Laboratory units must match the calculator, results should come from the same encounter, and treatment or chronic illness can change the values. Clinical features such as severe pain out of proportion, rapidly progressive swelling, skin discoloration, bullae, crepitus, systemic toxicity, hypotension, and altered mental status demand urgent attention regardless of score. Necrotizing soft tissue infection is a time-critical surgical emergency. The calculator must never postpone senior review, broad-spectrum antimicrobial therapy, resuscitation, imaging when appropriate, or operative exploration when suspicion is meaningful. Surgical assessment and direct tissue evaluation remain decisive. Use LRINEC only as one structured piece of information alongside history, examination, disease trajectory, comorbidities, imaging, and local emergency pathways.
