First, concentration equals the medication amount in the vial divided by the diluent volume, producing milligrams per milliliter. Second, the volume needed for a desired dose equals that dose divided by the calculated concentration. For example, a 1,000 mg vial combined with 10 mL gives 100 mg/mL. A desired 250 mg dose at that concentration occupies 2.5 mL. The arithmetic is simple, but safe reconstitution requires more information than arithmetic alone. The product insert, pharmacy label, institutional protocol, and prescriber order determine whether a product should be reconstituted, which diluent is compatible, how much liquid to add, the actual final volume, storage conditions, route, stability, and administration rate. Some powders displace liquid, so the final volume may differ from the volume of diluent added.
