About the pack-year calculator

A pack-year is a standardized way to describe cumulative cigarette smoking exposure. One pack-year represents smoking one conventional pack every day for one year. The calculation multiplies average cigarettes smoked per day by years smoked, then divides by the number of cigarettes in a pack. With a 20-cigarette pack, 20 cigarettes daily for 10 years equals 10 pack-years, while 10 cigarettes daily for 20 years also equals 10 pack-years. Pack-years help clinicians record smoking history consistently even when intensity and duration differ. They are commonly considered during assessment for chronic obstructive pulmonary disease, cardiovascular disease, lung cancer risk, respiratory symptoms, and eligibility for certain screening programs. The value is an exposure measure rather than a diagnosis or a direct probability. Two people with the same pack-year total can have different risks because age, genetics, occupational exposures, other tobacco products, time since quitting, and medical history also matter. Use a realistic long-term average if cigarette consumption changed over time. For a more precise history, calculate each period separately and add the results. For example, five years at 10 cigarettes per day and another five years at 20 per day should be calculated as two intervals. Former smokers should include all years during which they smoked; quitting does not erase historical exposure. Nevertheless, stopping smoking provides meaningful health benefits at any age and at every cumulative exposure level. Pack size defaults to 20 cigarettes because that is a common standard, but packages and hand-rolled cigarettes vary.
