About gout diagnosis assessment

Gout is an inflammatory arthritis caused by monosodium urate crystals in and around joints. A typical attack develops quickly, causes intense pain and swelling, and often affects the first metatarsophalangeal joint at the base of the big toe. Serum urate, previous episodes, visible or palpable tophi, and characteristic ultrasound or radiographic findings can strengthen clinical suspicion. No single symptom or blood result proves gout, so structured criteria help organize the evidence. This calculator assigns points to features represented in the legacy assessment: sex, serum uric acid, the primary joint involved, attack duration, the number of previous attacks, tophi, and imaging evidence. Higher serum urate, first-MTP or multiple-joint involvement, rapid onset, recurrent attacks, tophi, and positive imaging increase the score. A negative imaging result slightly lowers it. The percentage is a normalized presentation of this screening score and is not the probability generated by the formal 2015 ACR/EULAR classification criteria. The reference standard for a definitive diagnosis is identification of monosodium urate crystals in synovial fluid or material from a tophus using polarized microscopy. Joint aspiration also helps exclude septic arthritis, which can resemble gout and requires urgent treatment. A normal uric acid measurement during an acute flare does not reliably rule out gout because serum levels can temporarily fall. Conversely, hyperuricemia is common and many people with an elevated result never develop gout. Imaging may contribute when aspiration is unavailable or inconclusive. Ultrasound can identify a double-contour sign or tophus, while dual-energy computed tomography can detect urate deposition. Plain radiographs are more useful in established disease than during a first attack. The meaning of any finding depends on image quality, reader expertise, disease duration, and alternative diagnoses. Use this result to support, not replace, a clinician’s assessment. Sudden severe joint pain with fever, systemic illness, immunosuppression, a prosthetic joint, or concern for infection warrants prompt medical evaluation. Treatment decisions should account for kidney function, medicines, cardiovascular conditions, attack frequency, crystal confirmation when appropriate, and shared decision-making.

No. High uric acid supports suspicion but is neither necessary nor sufficient for diagnosis, especially during an acute flare.

What is the most definitive gout test?

Finding monosodium urate crystals in aspirated joint fluid or tophus material is the diagnostic reference standard. Aspiration can also identify or exclude infection.

Why is the big toe important?

The first metatarsophalangeal joint is a classic site of gout and increases this score. Gout can nevertheless affect ankles, knees, hands, and other joints.

What are tophi?

Tophi are deposits of urate crystals that usually develop after sustained crystal burden. Their presence strongly supports gout but is uncommon early in disease.

Does a low result rule out gout?

No. The score is a screening aid and incomplete data or an atypical presentation can lower it, so persistent clinical concern needs professional evaluation.
