About corrected calcium

Calcium in blood exists in several forms. Roughly half is biologically active ionized calcium, while much of the remainder is bound to albumin and other anions. A routine total calcium test measures all forms together. When albumin is unusually low or high, total calcium may look abnormal even though the ionized portion is not changed to the same degree. Albumin correction attempts to account for that binding effect. This calculator applies a common Payne-style equation in conventional units: measured total calcium plus 0.8 times the difference between 4.0 g/dL and the measured albumin. If albumin is below 4.0, the adjustment raises total calcium; if albumin is above 4.0, it lowers the estimate. Many laboratories use a typical total calcium reference interval near 8.5 to 10.5 mg/dL, but local ranges differ. Corrected calcium is an estimate rather than a direct measurement of physiologically active calcium. The original correction formulas were developed from particular patient populations and laboratory methods. Their accuracy can be limited in critical illness, advanced kidney disease, major acid-base disturbance, paraproteinemia, severe hypoalbuminemia, and other settings where calcium binding behaves differently. Some contemporary evidence favors measuring ionized calcium directly when the result will affect urgent care. Interpretation should consider symptoms, renal function, magnesium, phosphate, parathyroid hormone, vitamin D, medications, and the laboratory's own reference interval. Low calcium may accompany vitamin D deficiency, hypoparathyroidism, kidney disease, pancreatitis, or low magnesium. High calcium may be associated with hyperparathyroidism, malignancy, medication effects, dehydration, or other causes. A calculated result alone cannot determine the cause. Use values from the same blood sample and keep the units consistent. If your laboratory reports calcium in mmol/L or albumin in g/L, convert them before using this conventional-unit equation or use a formula validated for those units.
